Cardinal Hospice's Services


Cardinal Hospice provides services to patients in their private homes, Nursing Homes, Assisted Living, Respite Homes, and Independent Living Facilities.  We service patients in a 50 mile radius of Baton Rouge, including but not limited to areas such as: Denham Springs, Baker, Zachary, St. Francisville, Amite, Clinton, New Roads, Plaquemine, Donaldsonville, Pierre Part, and Lutcher.


Routine Home Care

Hospice care provided to patients in their homes or a home-like setting such as Nursing Home, Assisted Living Facilities, Independent Living Facilities, Respite Homes, and Group Homes.


Inpatient Care

Hospice Care provided to patients for short periods of time at contracted facilities when the patients symptoms can no longer be managed in the home.  The hospice team will continue to visit and oversee the needs of the patient.


​Continuous Care

​Hospice Care provided to patients on a continuous basis during a crisis situation, the hospice team will coordinate care with the family to provide symptom management to meet the patient's needs.


Respite Care

Respite Care is a temporary service which provides the caregiver and family a period of relief from daily care of their loved one.  The hospice team will coordinate these services with one of our contracted respite care facilities, where the patient will temporarily reside for the respite care period.


Physician Services

Physician services are part of our hospice patient's benefit.  Our Medical Directors will work closely with our patient's Primary Care Physician


Nurse Services

Our nurses make regularly scheduled visits to the patient according to the patient's needs to provide holistic care to our patients. They will also educate and provide support to the patient and caregivers.

Home Health Aide

Our Home Health Aides are all certified by the Louisiana Nurses' Aide Registry.  They are trained to provide personal hygiene care, activities of daily living, and homemaker services.  


Social Worker

Our social workers plays an important role in our patients' lives.  They educate our patients and caregivers and provide emotional support during this time of transition.  They also assist with end of life tasks, practical, insurance, and financial concerns, along with assisting with making final arrangements.


Chaplains

Our Chaplains offers our patients and caregivers a non-judgmental and compassionate presence an acceptance of different beliefs, cultures, and values.  


Medications

Medications related to the primary hospice diagnosis, for pain and symptom management, are covered under the hospice benefit and are part of our hospice plan of care.


Durable Medical Equipment​

Cardinal Hospice will provide basic medical equipment to meet the individualized care for our patients.  Some of these items are:  Wheelchairs, Electric Hospital Beds, Walkers, and Oxygen.


Medical Supplies/Wound Care

Medical Supplies and wound care will be supplied to our patients in accordance to their hospice diagnosis.
